Oui, il est encore utilisé (en déclin, toutefois).
C'est un terme péjoratif/méprisant, mais je ne dirais pas que c'est une 
insulte.

Je dirais que c'est une marque de rejet lié à un complexe d'infériorité 
(ou à un complexe de supériorité supposé de la personne ainsi désignée).
C'est un peu similaire au terme "parigot" utilisé par certains 
provinciaux pour désigner les Parisiens.
