Not quite (I hope I get this right...)
Any newsserver that wants to may carry a moderated goup. The posting
machinery is arranged so that articles posted to the moderated group are
first forwarded by email to a team of moderators appointed by that
particular newsgroup. I think (I'm not sure) that articles for posting
may also be sent directly to the moderators by email instead.
The moderators will approve or reject those articles according to
criteria set by the users of that newsgroup. If approved, the article
is then posted to one of the newsservers that carries that group and it
then propagates in the normal way.
It's common for well-behaved posters to be included on a whitelist so
that they can bypass the approval process. This reduces the load on the
moderators. New members of the group will usually serve a probationary
period before being whitelisted. There will generally be some means of
notifying the author of a rejected article the reason for rejection,
usually by email or sometimes on a web-page.
