In the first computer controlled aircraft, the programmers were told that 10% of them would be on the first flight. Engine start, takeoff, and flying went perfect, than as the plane neared the ground it rolled 90º just 100 feet from the ground. The pilot flicked off the computer, saved the plane, and landed. Later investigation showed that the ailerons had their positions initialized while the pilot was doing his flight control maneuvers. many of the programmers came out of the plane sickened to their stomachs.
