The knees are not even close to the shifters: <https://www.youtube.com/watch?v=V1-CyMzCwk4> I also don't see any attempt to pause pedaling before, during or after shifting. Here's what happens when one tries to shift using the knees: "Downtube Shifting with Knees" <https://www.reddit.com/r/cycling/comments/6cnyvb/downtube_shifting_with_knees/> "I'm 6' 2" and also ride a 1988 Cannondale with DT shifters. My knees are never less than 5 inches away from the shifters so there is no way I could do this. Especially clipped in." "I can't even reach my DT shifters with my knees while staying seated (or anyhow while clipped in.)" "I think I would have to reach hard to get that to work.. I'm trying to picture it and I'm not even sure my knee would reach without contorting."
