Could be a bearing problem (and I understand you ordered a 
replacement already. Could be a cup loose in the frame.

On square cranks, a loose bolt/arm makes noise (much less 
likely on a spline crank). I think you said you ensured 
pedals are tight in the arms, yes?

You seem to have eliminated chainring bolts and a front gear 
wire hitting the arm so that about covers 'crank rotation 
noises'.
