It doesn't say who nor what his role was in connection to the death. Perry had died of a ketamine overdose, with a level in his system high enough for general anaethesia during surgery. He was being treated by a psychiatrist and an anaesthesiologist; the latter was his primary care physician. He was on ketamine infusion therapy but his last treatment was a week and a half earlier. If he didn't get the ketamine from the anaethesiologist, then he got it from somebody else and deliberately chose to abuse it. That he'd been on the treatment for some time strongly suggests that he would have been aware of how much was safe. It's not possible to safely self-administer that extremely high dose that he took. The finding of death was still accidental, but given that he took the overdose himself and knew how excessively high the dose was, it should have been ruled a suicide.
