If a person has achieved great success, why do they need a medal?
More icing on the cake?

BTW, I believe Trump's reasoning was flawed...

Presidential Medal of Freedom - to individuals who have made
exceptional contributions to the security or national interests of
America, to world peace, or to cultural or other significant public or
private endeavors

Trump stated - "When you get the Presidential Medal of Freedom it is
usually for other things, like you have achieved great success."

I don't believe Trump's idea of "great success" is related to the
Presidential Medal of Freedom. "Great success" tends to be associated
with an individual's achievement(s)...I'm not aware of Andrew Carnegie
getting the Presidential Medal of Freedom.

Note the "Freedom" word
