Jim Williams published one years ago - it's covered in one of his Linear 
Technology application notes.

It isn't. One art per million puts the harmonic content about 120dB 
below the fundamental, and a few people claim to have done better.

Spectrum analysers and Fourier transform analysis can pull out that 
level of  harmonic content - as you really ought to know.
