I skimmed "All Systems Red" last night and believe that you are correct. 
  I did not find the throw through the door event.  I guess that the 
show and the book are going to have differences.

I saw it elsewhere that the entire first season is going to be "All 
Systems Red".  Lots of stretching and new events might be required to 
make that happen.
