Since the thread drifted to films with helicopters, it's notable actor 
Vic Morrow and two child actors were killed in a helicopter crash during 
the filming of 'Twilight Zone: The Movie' in 1982.  Morrow and one of 
the child actors were decapitated and the other child crushed under the 
helicopter wreckage.  Pyrotechnic movie effects were blamed for causing 
the crash.
